Every student athlete needs to be registered on the Final Forms website.  No athlete will be permitted to practice until registration is complete and a current physical is on file in the athletic office.  If you have any questions regarding this process, please contact Athletic Director Kristen Holt at 419-221-0366 ext. 2132
