Student Registration: All students attending Bath Local Schools and/or Apollo - both living within Bath Local School District and those accepted by Open Enrollment - must complete student forms in FinalForms annually. Parents are asked to log into your FinalForms account each May to complete forms for the coming school year. If you need to make changes to your contact information, such as a new address or phone number, or changes to your transportation information, we ask that you please make these changes in your Final Forms parent account.

OPEN ENROLLMENT FORM FOR THE 2025-2026 SY:

Open Enrollment applications for the 2025-26 school year are closed. We have already accepted our open enrollment students for this school year. This form is for parents to submit if they move out of Bath School District during this school year and want to request their child stay at Bath Schools. Open enrollment applications can be mailed to Lori Griffiths, Bath Administration Office 2650 Bible Road Lima, Ohio 45801 with one proof of new residency.

OPEN ENROLLMENT FOR THE 2026-2027 SY:

Bath Local Schools will begin accepting Open Enrollment applications for the 2026-27 school year on February 2nd, 2026. To apply for Open Enrollment for the 2026-2027 school year, parents must submit one application per student and one proof of residency to EMIS Coordinator Lori Griffiths. Parents are required to apply for open enrollment for each school year, regardless if the student already attends Bath Schools. Open enrollment applications will be accepted through May 1st, 2026 and can be mailed to Lori Griffiths, Bath Administration Office 2650 Bible Road Lima, Ohio 45801
