If you need to make changes to your contact information, such as a new address or phone number, or changes to your transportation information, please make these changes in your Final Forms parent account.
Open Enrollment for the 2023-24 school year is closed. We have already accepted our open enrollment students for this school year. To apply for Open Enrollment for the 2024-2025 school year, parents must submit one application per student and one proof of residency to EMIS Coordinator Lori Griffiths. Parents are required to apply for open enrollment for each school year, regardless if student already attends Bath Schools. Open enrollment applications will be accepted February 1st through May 1st, 2024 and can be mailed to Lori Griffiths, Bath Administration Office 2650 Bible Road Lima, Ohio 45801
