In accordance with the Ohio Department of Health, all incoming kindergarten students are required to have the following immunizations prior to the start of the school year:  DTap, Polio, MMR, Hep B and Varicella (Chicken Pox) 

In accordance with the Ohio Department of Health, all incoming 7th grade students are required to have the following immunizations prior to the start of the school year: TDap (Adacel or Boostrix) and Meningococcal (Menactra)

Vaccinations can be obtained from your family doctor or the Allen County Health Department.  Allen County Public Health will be seeing children and teens in need of immunizations by appointment only. They are prioritizing appointments by those in need of vaccinations for school attendance first.  Parents can schedule their child's appointment by calling 419-228-4457.
