The office will need updated contact information from you in written form to provide your transcript.
The information below must be included, in the body of an email, or on the attached form (transcript request form).

Grad Year/Years attended
Where you would like to have your transcript mailed, emailed or faxed
Applicable contact information
If you need the transcript for educational purposes, typically
the institution will need to have a hard copy mailed directly to them.
